Salary Range: $33.36-$47.06 per hour
This position will be eligible for Clay County’s benefit package.
- Bachelor’s degree from an accredited four-year college or university with a major in Social Work, Psychology, Sociology or closely related field.
- 1-3 years prior work-related experience
- Valid driver’s license
- Must be a current Clay County employee who meets the eligibility criteria for internal postings under the Clay County/Teamsters Local 120 labor agreement
- LSW Preferred
- Successfully complete a background investigation

- Complete MnChoices Assessments for individuals currently accessing Home and Community Based waivered services and state planned services.
- Coordinate with a person’s case manager and external providers to ensure a thorough and accurate assessment is completed.
- Complete required documentation by preparing and maintaining all case records.
- Maintain eligibility as a certified assessor by completing mandatory and non-mandatory training.
- Other duties as assigned.
