The Licensed Vocational Nurse supports client treatment and recovery by providing competent and compassionate nursing services, medication oversight, and accurate records management across both the residential (RTC) and outpatient programs. The LVN reports directly to the Outpatient Program Director and collaborates with clinical, psychiatric, and operations staff to ensure high‑quality, compliant care.
Essential Functions and Responsibilities :- Complete daily SOAP Notes at our RTC and three times per week at our Outpatient facility and ensure records are maintained in accordance with standards of facility and state regulations.
- Perform Urinalysis from Outpatient facility clients three times per week and report any concerns to the treatment team and psychiatrist.
- Observe clients, charting and reporting changes in condition (e.g., adverse medication reactions or clinical deterioration), and take appropriate follow‑up action.
- Observe and document clients self‑administering prescribed medications in accordance with medication management policy.
- Communicate with the facility’s medical provider (IMS Physician or designated physician) and other outside physicians when an external referral may be indicated.
- Coordinate with the physician and pharmacy to ensure medication, nursing, or health‑related concerns are promptly reported and resolved.
- Input and update medication protocols in Kipu EMR to ensure accuracy and compliance.
- Schedule client appointments with psychiatrists, PCPs, and other specialists; manage telehealth logistics (Zoom links, appointment readiness) and arrange transportation through the Facilities team when needed.
- Provide training and support to non‑clinical staff related to medication protocols and client supervision procedures.
- Measure and record clients’ vital signs (height, weight, temperature, blood pressure, pulse, respiration), and report significant findings to the physician, if not already documented by psychiatric technicians.
- Perform weekly inventory check and maintenance of nursing and OTC medication supplies.
- Maintain strict adherence to HIPAA regulations and CARF standards for documentation, safety, and client confidentiality.
- Uphold all facility policies and procedures regarding ethics, boundaries, infection control, and professional conduct.
- Participate in interdisciplinary team meetings as needed to support continuity of care and clinical collaboration.
- Assist with onboarding and training related to medication protocols and infection control for new staff.
- Serve as a point of contact for medical emergencies, ensuring appropriate first response and documentation.
Salary Range: $32.00 To $35.00 Hourly
RequiredEducation & Experience:
- Current & active Licensed Vocational Nurse license in the State of California.
- Current drivers’ license, current automobile insurance, and clean DMV report.
- Two‑year minimum experience working in a mental health treatment or behavioral health setting preferred.
- Experience with KIPU EMR is a plus.
