Demonstrates knowledge of skilled clinical nursing and works with the Intake Manager to coordinate the total care of the homebound patient. The position communicates with referral sources and provides an initial intake of agency referrals. The position may require home visits under the supervision of the Intake Manager or Director.
Duties/Responsibilities- Coordinates the referral process by taking the referral, evaluating the appropriateness of the referral and identifying other skilled care needs/community resources as needed, instructing referral sources regarding completion of the face-to-face encounters, and tracking completion of the encounters, assessing physicians for PECOS status as required, must have a good understanding of qualifications for payer sources to determine eligibility of the home health referral.
- Assigns cases to manager/requested disciplines.
- Communicates referral/intake activity as scheduled (to notify case manager/requested disciplines).
- Maintains the intake log Electronic Medical Record.
- Assists with management and assigning float employees.
- Assists with other office nursing duties, such as taking MD orders and coordinating changes with field employees.
- Makes home visits to assist as needed. Performs nursing skills and procedures as defined and authorized by KBN and under the direction of an RN or Physician.
Graduate from an approved Practical Nursing Program required.
ExperienceHome Health experience preferred.
Licensure/CertificationValid Kentucky Board of Nursing (KBN) License required. Valid Kentucky Driver’s License required.
