Pay and Locations

Hourly rate: $27–$32, case dependent.
Locations: Moreno Valley, Corona, Chino, Chino Hills, Riverside.

Shift availability: Flexible. Urgent need for full‑time PM/overnight and AM weekend nurses.

Position Overview

The Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care under the supervision of a Registered Nurse, following the care plan and physicians orders while maintaining confidentiality and professional boundaries.

Essential Job Functions

• Deliver and coordinate quality patient care per physician orders.
• Continuously observe and assess patient condition and report changes to supervisor/physician.
• Document all activities, assessments, nursing actions, responses and coordination of care promptly.
• Participate, implement, and update the nursing care plan.
• Take appropriate nursing actions based on assessment to achieve expected outcomes.
• Recognize changes in patient needs and intervene to prevent or reduce risk.
• Accept responsibility for personal and professional accountability, complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
• Use infection control measuresباحية according to OSHA standards.
• Educate patient and family on disease process, self‑care, and prevention strategies.
• Maintain nursing competence through education, internal learning management, and skill evaluations.

Qualifications

• Active, unencumbered CA LVN license.
• Current CPR certification with hands‑on component.
• TB skin test (two‑step) current within the last 12 months.
• One year of prior hands‑on nursing experience.
• Reliable transportation.
• Continuing education required by state.

Additional State Specific Requirements

• South Carolina – One (1) year of tungsten experience.
• California – One (1) year of experience under current nursing license.
• Louisiana – One (1) year of experience under state license.
• Continuing education as required by state.

Preferences

• Six (6) months recent experience as a Licensed Nurse in a clinical setting.
• Home‑health experience.

Physical Requirements

• Speak, write, read, and understand English.
