CASE MANAGER DUTIES AND RESPONSIBILITIES

South mountain Children and Family Services is seeking a compassionate, experienced Case Manager to join its dedicated team in the Residential Care Program. The Case Manager ensures that the case management and family support needs of children in care are met to the highest standards. This role involves providing intensive social work services to children and families navigating complex situations, to support reunification and long-term stability.

The position is full-time, 40 hours per week, typically Monday through Friday, but requires flexibility for after-hours emergencies and on-call rotation.

QUALIFICATIONS
  • Bachelor’s Degree in Social Work or a related human services field required;
    Master’s Degree preferred
  • Two or more years of experience providing case management or social work services preferred
  • Experience working with children and families involved in the child welfare system is strongly preferred
  • Knowledge of trauma-informed practices and family reunification processes
  • Strong communication, documentation, and organizational skills
DUTIES
  • Provide intensive case management services for children residing in South mountain’s Residential Care Program.
  • Deliver direct social work services to children and families to support reunification or permanency efforts.
  • Develop and maintain a resource guide for each county served.
  • Collaborate with staff at the Foster Community, Journeys Home, Family Centered Treatment Practitioners, and other program partners to address Family Service Plan objectives and support reunification or permanency efforts.
  • Participate in on-call rotation for family emergencies and crises.
  • Advocate for families, connecting them with appropriate community resources and informal supports to strengthen family self-efficacy.
  • Build and maintain relationships with community resources, interagency networks, and informal family supports.
  • Demonstrate strong interpersonal skills and the ability to work both independently and collaboratively with others.
  • Assess risk and safety factors for children and families and participate in Shared Parenting meetings as needed.
  • Attend relevant trainings to enhance family skill-building and support professional development, including ongoing training, peer consultation, and agency events that promote growth, public awareness, and community engagement.
  • Maintain regular communication with county social workers regarding family progress and case plan goals.
  • Participate in team meetings, IEPs, MDTs, CFTs, and court proceedings as required.
  • Track and coordinate medical, dental, and therapeutic appointments for children in care and in permanent placements.
  • Maintain timely and accurate documentation in electronic records.
