At Easterseals Port Health, we bring care directly to clients, providing support in their homes, or other community settings. You’ll form meaningful relationships and see the immediate impact of your work. Working on an ACT Team means being part of a multidisciplinary approach that prioritizes holistic care and client-centered support.

• Community Impact:
  Deliver vital support in real-world settings, helping individuals regain independence.
• Collaborative Care:
  Work alongside psychiatrists, nurses, co-occurring disorder specialists, and other mental health professionals, ensuring clients receive comprehensive, integrated care.
• Advocacy with Integrity:
  Champion clients’ rights and preferences, ensuring that their voices are heard and respected.
• Professional Growth:
  Receive up to $65/week for licensure supervision, ongoing training, and career development opportunities.

• Provide individual and group therapy to 4-5 adult clients daily in the community.
• Develop individualized treatment plans and manage client care with the ACT team.
• Consult and collaborate with community agencies and families to maintain coordination.
• Participate in daily team meetings to ensure quality service delivery.
• Maintain accurate and timely documentation using Welligent EMR.
• Offer on-call crisis intervention services as part of a rotating schedule.

• Licensed or provisionally licensed in NC (LCSW/A, LPC, LMFT/A, LCMHC/A).
• Comfortable working in community settings with a valid driver’s license.

• Hourly Salary: $52,000 to $65,000/year | $25.00 – $31.25/hour, based on licensure and experience
