• Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the unique needs of our residents.
• Provide hands-on restorative care, including range of motion exercises, transfer techniques, and positioning strategies to promote comfort, independence, and optimal functioning.
• Conduct thorough assessments to identify areas for improvement and develop strategies for improvement.
• Communicate effectively with residents, families, and healthcare professionals to ensure continuity of care and address concerns.
• Participate in ongoing education and professional development to stay current with best practices and advancements in restorative care.
• Maintain accurate records and reports, including resident assessments, care plans, and progress notes.
