Population Health Manager (RN) – ACO – Morristown
Base pay: $48.00/hr – $84.50/hr
Job Description
Responsible for identifying population health needs and facilitating best practice service delivery to achieve healthy lifestyles and better health outcomes for the population.
Principal Accountabilities
- Identifies population health needs, maps services and identifies gaps in healthcare services.
- Engages relevant stakeholders (internal and external) to develop and implement service delivery requirements.
- Establishes and maintains strategic and program alliances with other agencies whose activities impact public health.
- Facilitates and/or contributes to models and plans meant to optimize capacity to meet the healthcare needs of the region.
- Collates and analyzes population data from various databases including state and national to support local planning.
- Utilizes routinely and purpose‑specific collected data to describe and summarize the health status of the population to identify priorities for public health action.
- Other duties as assigned.
- Travel required to physician practices.
Qualifications
- Bachelor’s degree in nursing required.
- Current NJ state licensure as a Registered Nurse required.
- 3 years of similar experience in a hospital setting preferred.
