Outreach Representative

Job Description

Under the direction of the VP of Community Engagement, the Outreach Representative will identify individuals and families in need of health care services, family planning education and link them to ZHC. Outreach will mainly be conducted through Community Based Organizations, churches, schools, local government, and non-government agencies to increase awareness of Zufall health services and increase referrals to Zufall.

• Assist in obtaining essential support services such as Medicaid, food stamps, transportation, translation services and day care.
• Provide cancer education awareness/CEED services throughout Somerset County.
• Serve as an advocate for patients and act as liaison between patients and ZHC.
• Attend in-service education and training programs.
• Provide basic health information and education to patients.
• Arrange referrals and coordinate transportation as needed.
• Assist medical staff in care management through outreach using patient registries.
• Provide ongoing social and emotional support through care management.
• Complete daily activity logs, referral forms and other reports as necessary.
• Provide sexual health education topics to students and other community members throughout ZH service area.
• When presenting to schools, arrive between 7–7:30 AM and remain until about 2:30 PM.
• Assist in monthly data scrubbing – search for missing data in provider charts and correct monthly reports for both programs.
• Assist manager with other duties as time permits depending on school presentation schedule.
• Assume other program related tasks as assigned.

• Ability to communicate in English, both orally and in writing; speak Spanish fluently.
• Ability to operate a motor vehicle and travel within the ZHC service site areas.
• Strong interpersonal and communication skills, ability to demonstrate empathy.
• Ability to relate to patients and to local government and community organization workers and leaders.
• Work independently; seek assistance when necessary.
• Works with medical and social work professionals in a multi‑disciplinary team, in a fast‑paced environment.
• Basic knowledge of target populations’ ethnic and medical needs.
• Flexibility, cultural sensitivity, and tactfulness.
• Maintain HIPAA compliance in all regulated transactions and comply with all regulatory standards.
• Comfortable with public speaking.
• Must be able to talk about sex, contraceptive methods, sexually transmitted infections, sexual/reproductive/pregnancy coercion – training will be provided on these topics.

• High School diploma or equivalent.
• Bilingual in English and Spanish (required).
• Comfortable with public speaking on topics regarding sex, STIs, and pregnancy coercion (required).
• Previous experience in outreach preferred.

$21.00–$24.34 hourly
