Licensed Practical Nurse (LPN) – Skilled Nursing Facility

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide exceptional care and contribute to a supportive and professional environment.

• Deliver direct nursing care to residents in accordance with the care plan

• Administer medications and treatments as prescribed

• Monitor residents' conditions and report changes to the Registered Nurse or physician

• Document resident care accurately and timely

• Supervise Certified Nursing Assistants (CNAs) and ensure care plans are followed

• Assist with admissions, transfers, and discharges

• Communicate effectively with residents, families, and interdisciplinary team members

• Maintain infection control and safety standards in accordance with facility policies

• Active LPN license
