Job Description

Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a full‑time GIP hospice Registered Nurse to work Monday – Friday from 8 AM to 4 PM in the Morris, Essex, Union, and Somerset County area, including weekends and on‑call as required by department. In this position you will work with adult and geriatric patients of all backgrounds who require hospice care at one of our hospital sites, sometimes in their homes as needed.

You are compassionate, thoughtful, and comfortable working with elderly and terminally ill patients. You will be responsible for managing a heavy caseload (up to 4 patients a day); time‑management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs.

Job Qualifications / Experience
  • Current NJ RN License
  • NJ driver’s license in good standing
  • Associate’s Nursing Degree
  • BLS certification
Preferred
  • Minimum of one‑year experience in acute, sub‑acute or home/hospice care setting
  • Experience using an EMR, excellent critical thinking and communication skills
Duties & Responsibilities
  • Perform patient assessments and accurately assess patient’s appropriateness for hospice care, learning needs and progress toward goals
  • Develop the plan of care for the patient and family
  • Ensure appropriate utilization of services and maintain visit productivity standards
  • Demonstrate knowledge of insurance reimbursement and hospice regulations
  • Complete discharge and admission assessments, including data transfer, within required time frames
  • Use age‑appropriate teaching methods for patients and families, evaluate teaching plans and document education tools used
  • Assume responsibility for case management: develop, implement and revise plan of care based on evidence‑based knowledge, prioritize care needs, demonstrate interdisciplinary communication, perform HHA supervisions per standards, and follow scheduling process
  • Maintain a safe environment for patients and co‑workers: follow infection control measures, demonstrate pharmacology knowledge, reconcile medications, and maintain current medication orders
  • Attend monthly staff meetings and periodic performance improvement meetings, and accept other assignments as requested
  • Participate in performance improvement program: review health records, gather data, attend UR meetings, provide orientation/mentorship to new employees, cross‑train to learn new skills
  • Assist with growing agency business by providing orientation, accepting assignments, and cross‑training
