Overview
Position will be a dedicated Hospice Educator, providing education and mentoring to hospice nurses in a classroom and field setting while maintaining skills by visiting patients in their home
Duties & Responsibilities- Maintain a thorough knowledge of the Medicare/Medicaid Hospice benefit including admission criteria, philosophy and services.
- Educate, mentor and precept Hospice clinical staff to assist them in being successful and knowledgeable in their roles.
- Maintain current knowledge of clinical issues, regulatory and reimbursement standards.
- Evaluates performance of new staff and modifies teaching and orientation plans/milestones in coordination with the Education Team.
- Evaluates, teaches and mentors clinicians in Hospice case management and interdisciplinary care planning.
- Completes team member evaluations at end of introductory period.
- Participates in the development of staff performance management/improvement plans, including monitoring and improvement strategies.
- Assesses and monitors clinical competency of assigned staff during introductory period and ongoing.
- Collaborates with Assistant Managers to support:
- Daily Scheduling
- Staff and Patient problem solving
- Cross training and coverage of Assistant Managers, staff & Hospice CRC:
- Duties assigned to Assistant Managers
- POC review
- Review Admission/Recert visits
- Hospice Admission Coding
- CTI completion
- Work Que management
- Reviews visit documentation including:
- Assessments
- Care Plans
- Medications
- POC
- IOT notes
- Field Expectations:
- Maintains a minimum productivity of X # of visits per week which may include:
- Joint Visits
- Overflow of Admissions
- Managing Caseload
- Additional Coverage as needed
- Reviews visit documentation including:
- Assessments
- Care Plans
- Medications
- POC
- IDT Notes
- Attends mandatory meetings as required.
- Current NJ RN License
- Associates in Nursing
- BSN preferred
- CHPN preferred
- 3-5 years' experience as an RN in Hospice/Palliative Care or related field
- 2-3 years' experience in Hospice supervision/education preferred
- Pain and symptom management skills
- Knowledgeable in the signs and symptoms associated with the end of life
- Excellent computer skills, problem solving skills
- EMR experience, EPIC preferred
- Minimum Salary (Hourly Rate): 48.000000
- Maximum Salary (Hourly Rate): 84.500000
