Atlantic Visiting Nurse (Part‑time Hospice RN)

Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a part‑time hospice Registered Nurse to work 4 PM to 2 AM in the Morris County area, including weekends and on‑call as required. The RN will provide hospice care to adult and geriatric patients, often in their homes. The role requires strong time‑management skills with a caseload of approximately four patients per day.

Duties & Responsibilities
  • Accurately assess patient appropriateness for hospice care, learning needs, and progress toward goals.
  • Develop the plan of care for the patient and family.
  • Ensure appropriate utilization of services.
  • Maintain visit productivity standards.
  • Demonstrate knowledge of insurance reimbursement.
  • Complete discharge assessments.
  • Perform laptop transfer of data twice daily.
  • Complete admission assessment and transfers within 24 hours.
  • Complete re‑visit documentation and transfers.
  • Use age‑appropriate teaching methods for patients and families.
  • Evaluate teaching plan and revise as necessary.
  • Document patient education tools used.
  • Assume responsibility for case management.
  • Develop, implement, and evaluate plan of care based on evidence‑based knowledge.
  • Revise care plan as indicated.
  • Prioritize care needs.
  • Demonstrate knowledge of hospice regulations and insurance reimbursement guidelines.
  • Show evidence of interdisciplinary communication.
  • Perform HHA supervisions per standards.
  • Follow scheduling process.
  • Communicate progress toward goals with IDT.
  • Attend and participate in IDT meetings every 2 weeks.
  • Evaluate the plan of care and revise as needed.
  • Maintain a safe environment for patients and co‑workers.
  • Follow infection control measures and report infections.
  • Demonstrate knowledge of pharmacology and safe medication management.
  • Reconcile medications routinely.
  • Maintain current medication orders.
  • Attend monthly staff meeting.
  • Participate in PI meetings and activities/committees as requested.
  • Accept other assignments as requested.
  • Prioritize schedule to accommodate patient’s needs.
  • Document case management activities.
  • Review health records and gather data to support PI activities.
  • Attend UR meetings as requested.
  • Provide orientation/mentorship to new employees.
  • Cross‑train to learn new skills and demonstrate willingness to teach others.
  • Assist with growing agency business.

Qualifications for Internal Candidates

Required:

  • Graduate of an accredited Nursing Program – Associate’s Degree.
  • Current state licensure as a Registered Nurse.
  • BLS certification.

Preferred:

  • Bachelor’s degree in nursing.
  • All ASN must complete BSN studies within five years of start date.
  • Master’s degree in nursing or related field.
  • 1+ year of experience in nursing or related area.
  • ACLS certification.
