Licensed Practical Nurse (LPN) – Skilled Nursing Facility
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide exceptional care and contribute to a supportive and professional environment.
Responsibilities:- Deliver direct nursing care to residents in accordance with the care plan
- Administer medications and treatments as prescribed
- Monitor residents' conditions and report changes to the Registered Nurse or physician
- Document resident care accurately and timely
- Supervise Certified Nursing Assistants (CNAs) and ensure care plans are followed
- Assist with admissions, transfers, and discharges
- Communicate effectively with residents, families, and interdisciplinary team members
- Maintain infection control and safety standards in accordance with facility policies
- Active LPN license
