Atlantic Health is hiring a Hospice GIP Registered Nurse to work 12 PM – 7:30 PM, full time, in Morris, Essex, Union, and Somerset Counties, including weekends and on-call as required by department.

Base pay range

$40.00/hr - $70.40/hr

Job Description

Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a full-time GIP hospice Registered Nurse to work 12 PM to 7:30 PM in the Morris, Essex, Union, and Somerset County area, including weekends and on-call as required by department. In this position you will work with adult and geriatric patients of all backgrounds who require hospice care at one of our hospital sites, sometimes in their homes as needed.

You are compassionate, thoughtful, and comfortable working with elderly and terminally ill patients. You will be responsible for managing a heavy caseload (up to 4 patients a day); time management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs.

Responsibilities
  • Performs patient assessments.
  • Accurately assesses patient’s appropriateness for hospice care, learning needs and progress toward goals.
  • Develops the plan of care for the patient and family.
  • Ensures appropriate utilization of services.
  • Maintains visit productivity standards.
  • Demonstrates knowledge of insurance reimbursement.
  • Completes discharge assessments.
  • Performs laptop transfer of data twice daily.
  • Completes admission assessment and transfers within 24 hours.
  • Completes re-visit documentation and transfers.
  • Uses age-appropriate teaching methods for patients and families; evaluates and revises teaching plans and documents patient education tools used.
  • Assumes responsibility for case management; develops/implements/evaluates plan of care based on evidence-based knowledge; revises care plan as indicated; prioritizes care needs; demonstrates knowledge of hospice regulations and insurance reimbursement guidelines; shows evidence of interdisciplinary communication; performs HHA supervisions per standards; follows scheduling process; communicates progress toward goals with IDT; attends IDT meetings every 2 weeks; evaluates the plan of care and revises as needed.
  • Maintains a safe environment; follows infection control measures; demonstrates knowledge of pharmacology and safe medication management; routinely reconciles medications; maintains current medication orders.
  • Attends monthly staff meetings; participates in PI activities/committees as requested; accepts other assignments as requested.
  • Participates in performance improvement program; reviews health records; attends UR meetings; provides orientation/mentorship to new employees; cross-trains to learn new skills and teach others.
  • Assists with growing agency business; provides orientation to new employees; cross-trains to learn new skills.
Qualifications
  • Graduate of an accredited Nursing Program - Associate’s Degree.
  • Current state licensure as a Registered Nurse.
  • BLS certification.
Preferred
  • Bachelor’s degree in nursing.
  • All ASN must complete BSN within five years of start date.
  • Master’s degree in nursing or related field.
  • 1+ year of experience in Nursing or relevant area.
  • ACLS certification.
