American Traveler is hiring an RN Case Manager for inpatient discharge planning with at least 2 years of acute care hospital experience required.

• Inpatient Care Management unit within a hospital setting
• Coordinates patients through acute care hospitalization and manages discharge planning and transition processes
• Performs utilization management and quality screenings
• Works with subacute patient populations
• AM shift schedule, 8:00 AM to 4:00 PM, 37.5 hours per week
• Rotating weekends required approximately every 4th or 5th weekend
• Floating to a nearby sister hospital as needed
• No on-call responsibilities
• Uses Epic for charting

• Active NJ or compact RN license required
• BSN required
• CCMC certification preferred
• Minimum 2 years of acute inpatient hospital discharge planning experience required
• Hospital experience with discharge planning and placement in rehabilitation facilities required
• Experience with subacute patients required
• In-Patient Case Management experience required

• Coordinates all aspects of discharge planning and transition of care for acute hospital patients
• Holiday coverage required for one summer and one winter holiday
• Local candidates are accepted
• Travelers may extend assignments up to one calendar year from initial start date before requiring a break
