American Traveler is hiring an experienced RN Case Manager for inpatient hospital discharge planning, requiring a NJ or compact license and prior acute care discharge planning experience.

• Care Management unit within an acute care hospital setting
• Coordinates patients through acute care hospitalization and manages discharge planning and transition processes
• Performs utilization management and quality screenings
• Experience with subacute patient populations required
• Day shift schedule, 8:00 AM to 4:00 PM
• 37.5 hours per week, 7.5 hours per day, 5 days per week
• Rotating weekends required every 4th or 5th weekend
• Float as needed to sister hospital location
• No on-call responsibilities
• Uses Epic EMR

• Active RN license (NJ or Compact) required
• BSN required
• CCMC certification preferred
• Minimum 2 years of acute inpatient hospital discharge planning experience required
• Hospital experience including discharge planning and placement in rehabilitation facilities required
• Case Management and Managed Care experience required
• In-Patient Case Management experience required

• Coordinates all aspects of patient discharge planning and care transitions across the continuum
• Holiday coverage required for one summer and one winter holiday
• Local candidates accepted
• Assignments limited to one calendar year from initial start date, with a required break before returning
