OverviewMake a difference for Vermonters with complex health and social needs, working with community based teams and systems, as an RN Nurse for the Vermont Chronic Care Initiative (VCCI) with the Agency of Human Services (AHS). Have the work/life balance you need with a Monday‑Friday schedule and a supportive hybrid work station environment. The State of Vermont has an immediate opening for a Nurse Case Manager.
Eleven paid state holidays, in addition to a robust benefits package. Loan repayment may be available via federal/state eligibility requirements.
- Patient centered clinical case management to support health and health related outcomes for individuals with complex health & social needs.
- Community based case management and field based visits in private apartments, motels, correctional facilities, encampments, and shelters; reports to the Central Office Administration, collaborates with AHS Field Director, and works closely with partners and providers in the health and social services arena.
- Service delivery using a team based care approach, patient engagement tools, shared care plans, a strength based approach and motivational interviewing skills.
- Effective communication and working relationships with a diverse group of stakeholders, including the individual, family members and various health care disciplines and service providers; operational integration with other health care reform initiatives.
- Clinical experience and knowledge, knowledge of community, and community culture are necessary.
This position primarily covers Lamoille County, but flexibility to cover other counties may be requested.

Environmental FactorsDuties are performed in a facility, standard office setting and/or in the community. Extensive travel may be required, for which private means of transport must be available. Some functions may require duty outside of normal work hours.
Minimum Qualifications- Licensure as a Registered Nurse (RN) in Vermont or eligible to practice via a multi‑state license.
- Three (3) years or more professional nursing experience in an acute hospital setting, long term care, health insurance carrier, or within a community health/public health setting.
- Must maintain Vermont or multi‑state licensure as a Registered Nurse as a condition of employment.
- Bachelor of Science in Nursing (BSN).
Reliable means of personal transportation is required for work out of the office or in the community and field based settings.
