Position: LPN (Licensed Practical Nurse)

LPN (Licensed Practical Nurse) Skilled Nursing Facility
Full-time, Contract 13 weeks 2pm-10:30pm 2 on 1 off

Responsibilities
  • Provide direct patient care under the supervision of registered nurses.
  • Administer medications and monitor patient responses.
  • Participate in ongoing professional development and training.
  • Assist in developing and implementing patient care plans.
  • Collaborate with healthcare team members to optimize patient outcomes.
  • Engage in performance measurement and improvement initiatives.
  • Maintain accurate patient records and documentation.
  • Monitor vital signs and report changes to the nursing staff.
  • Ensure compliance with healthcare regulations and standards.
  • Educate patients and families on health management and care plans.
  • Assist with daily living activities for patients as needed.
  • Foster a positive and supportive environment for patients and staff.
Experience/Qualifications
  • Valid LPN license in the state of employment.
  • Commitment to ongoing professional development and training.
  • Experience in a clinical setting, preferably in patient care.
  • Ability to collaborate with healthcare team members for optimal outcomes.
  • Skills in educating patients and families on health management.
  • Strong communication skills for effective patient and team interactions.
  • Proficiency in documenting patient care and maintaining records accurately.
  • Ability to monitor vital signs and report significant changes promptly.

    Knowledge of healthcare regulations and compliance standards.
  • Willingness to assist patients with daily living activities as needed.
