HOME CARE SPEECH LANGUAGE PATHOLOGIST (SLP) – Per Diem, PRN

Liberty Home Care is dedicated to helping patients recover at home after illness, trauma, or surgery to improve outcomes and promote independence. We are currently seeking an experienced Speech Language Pathologist (SLP) to join Liberty Home Care.

• Provides patient care services along with direct treatment and follow-up.
• Evaluates and treats patients with a physician’s order for speech, language, swallow, or cognitive disorders, and develops a plan of care appropriate to the patient’s diagnosis.
• Documents patient care data accurately, thoroughly, and in a timely manner, reflecting evidence of improvement/progress/decline in the patient’s condition.
• Communicates with intercompany and intracompany contacts; maintains open lines of communication among branch administrative and management staff and other clinical staff.
• Attends patient care conferences, meetings, and mandatory in-services as scheduled; provides weekly statistical data.
• Provides in-service education, healthcare professional education, and public education; supervises peers and supporting personnel and students in training as needed by the company.
• Conducts research and assists with orientation of new employees as needed.

• Master’s degree in speech language pathology and Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA), plus current state licensure where employed.
• Strong knowledge of theory and concepts in speech-language pathology; ability to evaluate and treat patients with physician orders.
• Strong communication (oral and written) and organizational skills; CPR certification.
• Minimum of twelve hours of continuing education annually.
• Ability to travel and work as assigned.
• 1 year of therapist experience in any setting.
