Position SummaryThe primary purpose of this role is to return patients to optimal physical function in a skilled nursing facility (SNF). The SLP will facilitate and coordinate patient, physician, and therapy goals, teach patients to maintain functional gains after discharge through education, independent exercise programs, and activity modification.
Responsibilities- Collaborate with physicians, nurses, occupational and physical therapists to develop and implement individualized speech therapy plans.
- Conduct assessments, diagnoses, and interventions for patients with speech, language, voice, and swallowing disorders.
- Educate patients and families about communication strategies and post‑discharge self‑management.
- Document progress, produce reports, and ensure compliance with facility and state protocols.
- Master’s Degree in Speech‑Language Pathology from an accredited institution.
- Active, unencumbered state license to practice speech pathology.
- Basic computer skills, proficiency with service‑delivery software and electronic health record systems.
