• Give hands‑on care to patients nearing the end of life, following state nursing rules.
• Follow the care plan after assessing the patient's needs.
• Keep clear and timely records of the care provided.
• Teach and support patients and their families about the illness and care plan.
• Help coordinate care by communicating with the hospice team (aides, chaplains, social workers).
• Manage pain and symptoms, checking how well treatments are working.
• Take part in training and quality improvement activities to grow professionally and improve care.

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
• Currently licensed as an LPN/LVN in the State.
• Comply with accepted professional standards and practice.
• Must be a licensed driver with an insured automobile in good working order.
• Possess and maintain current CPR certification.
• Understand the philosophy and concept of hospice care; also understand the needs of the terminally ill.
• Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
• Demonstrate good verbal and written communication, and organization skills.
