We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled rehab facility in Mt. Lebanon, Pennsylvania. About the Role:As an LPN, you will be responsible for providing high-quality patient care in a collaborative and supportive environment. You will work closely with our interdisciplinary team to develop and implement individualized care plans, monitor patients' progress, and provide education and support to patients and their families.
Responsibilities:- Provide direct patient care, including administering medications, wound care, and respiratory treatments
- Develop and implement individualized care plans in collaboration with the healthcare team
- Monitor patients' progress, report changes, and adjust care plans as needed
Educate patients and their families on disease management, medication, and treatment - Document patient information accurately and thoroughly
- Participate in interdisciplinary team meetings to discuss patient care and treatment plans
- Maintain a safe and clean environment, adhering to infection control policies and procedures
