Physical Therapy Assistant – PACE Central Michigan

Program of All‑inclusive Care for the Elderly (PACE) is a unique health care model designed to help seniors stay safely in their homes for as long as possible. PACE coordinates and provides all medical care for its participants, and services are delivered across various settings, including on site at the PACE center, in the community, and in participant homes.

Employees have the opportunity to see the full impact of their efforts. Staffing ratios are much lower, time spent with each participant is greater, and team collaboration occurs more frequently than in other care settings. PACE is a very fulfilling health care delivery model for employees.

Support the physical therapy needs of PACE Central Michigan participants. The PTA assists the Physical Therapist in providing physical therapy treatments, procedures, and wellness programs in the home and at the PACE center. Provides input to support the development and modification of participant‑centered plans of care. Implements both one‑on‑one and group activities to assist participants to meet and/or maintain their functional and mobility goals.

• Provide treatment to participants as designed by the PT.
• Consult with PT regarding need to modify treatment plan and goals.
• Measure participants’ range of motion and vital signs to determine the effect of treatment.
• Provide rehab training for employees, PCA’s, and contracted staff.
• Confers with the physical therapist and other members of the IDT.
• Perform skilled modalities.
• Provide training to caregivers, family members and friends related to the functional mobility of participants following their goals and the organizational mission.
• Monitor and document routine maintenance of rehab equipment.
• Assess and provide education and training for participants with braces, prosthesis, splints, diabetic shoes and other supportive devices.
• Responsible for ordering DME and documenting in the medical record.
• Make quick repairs for DME as knowledge and skills permit.
• Must care, support and collaborate with everyone involved in the participant’s care including staff and the participant’s family using the participant centered care model.
• Participate in quality improvement activities as needed.

• Associate’s degree in the Physical Therapy Assistant Program.
• Graduate of a Physical Therapy Assistant (PTA) Program.
• Current, unencumbered active license to practice as a Physical Therapy Assistant (PTA) in the State of Michigan.
• Minimum of one year of experience working with the frail, elderly, or long‑term care population.
• Valid MI Driver’s License.
• Reliable transportation, current vehicle registration and insurance.
• Collaborate effectively with other IDT members.
• Current CPR certification by AHA or willing and able to obtain as offered by PACE Central Michigan.
