• Nurse Practitioners provide primary and/or consultative professional care in a variety of care settings, functioning in collaboration with physicians according to a Collaboration Arrangement.
• Focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life.
• Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in patients’ home environments.
• Determine follow up assessments according to patient needs and best practice guidelines.
• Communicate significant findings and changes to the clinical team, patients, families, facilities and other care personnel.
• Assess health status by obtaining relevant health and medical history and perform physical exams as per the Standard Care Arrangement.
• Order, interpret and evaluate laboratory tests and other diagnostic studies.
• Prescribe and monitor medications as allowed by prescriptive authority and state regulations.
• Develop treatment plans in collaboration with patients and caregivers.
• Refer patients to physicians or specialists as appropriate.
• Ensure continuity of care by liaising with the multidisciplinary care team.
• Provide patients and families with understanding of diagnosis and progression to reduce unnecessary ER and hospital visits.
• Hold case conferences with families when appropriate to advance the care plan.
• Meet minimum productivity requirements based on practice standards.

• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate).
• Master's or doctoral degree as a Board-C Certified Nurse Practitioner.
• Current State License as a Registered Nurse and Nurse Practitioner.
• Nursing skills as defined by accepted standards.
• Ability to sit, stand, bend, lift and move intermittently; ability to lift at least 35lbs.
• Current and valid driver's license with reliable transportation and auto insurance.
• Demonstrates ability to assess and respond to needs of patients, families and caregivers in skilled-nursing facility settings.
• Ability to handle multiple priorities, documentation requirements and deadlines.

• 2 years of clinical experience in a post-acute setting or as a Nurse Practitioner (preferred).
• Experience with geriatric patients (preferred).
• Familiarity with Aprima EMR (preferred).
