Registered Nurse RN Home Health

Overview

Elara Caring is hiring a Registered Nurse Home Health to deliver high‑quality, 1:1 patient care in the home setting. The role covers patients in Mt. Pleasant, Alma, Ithaca, Carson City, Ashley, Breckenridge, and St. Louis, MI.

• Assess and document the physical, psychosocial, and environmental needs of home health patients and families.
• Plan, coordinate, implement, and evaluate individualized plans of care.
• Conduct initial evaluations and re‑evaluations during visits, and make follow‑up visits as needed.
• Communicate significant findings, problems, and changes in health status or unsafe conditions to the Clinical Supervisor, physician, facility, and other team members.
• Maintain patient and staff privacy and confidentiality in compliance with HIPAA.
• Ensure all activities align with Elara Caring’s mission and quality standards.

• Current, unrestricted RN license in the state of Michigan.
• Minimum 1 year of recent clinical nursing experience in a patient‑care setting.
• Strong clinical assessment and care planning skills.
• Valid driver’s license, dependable vehicle, and current auto insurance; willingness to travel up to 50% of coverage area.
• Bachelor’s or Associate’s Degree in Nursing preferred.
• Prior Home Health, Hospice, or Home Base Home Care experience preferred.
