Overview
NIVA Health is hiring a Wound Care Provider in the Saginaw area to deliver advanced wound care through a facility-based rounding model. You'll work in partnered clinical settings such as skilled nursing facilities, rehab centers, and assisted living communities, collaborating with on-site care teams to improve healing outcomes and patient quality of life. This role is a great fit for a clinician who enjoys clinical autonomy, a consistent workflow, and working alongside established facility staff.
Responsibilities- Provide comprehensive wound assessments and treatment plans
- Deliver advanced wound care services, including dressing selection, treatment protocols, and ongoing monitoring
- Perform debridement and other procedures within your scope and credentialing (training and support available)
- Coordinate care with facility nursing teams and leadership
- Document visits clearly and efficiently in our system
- Communicate recommendations and follow-up plans with the care team
- Partnered facilities in the Saginaw area (SNF, rehab, assisted living, and similar settings)
- Structured rounding schedule based on patient needs and facility census
- Supported onboarding, clinical guidance, and operational coordination
- Active Nurse Practitioner/Physician Assistant (NP/PA) license
- Strong independence, communication, and organizational skills
- A passion for patient-centered care
- Clinical experience in wound care, SNF/rehab, home health, or related settings is a plus
- Strong clinical judgment and communication skills
- Reliable transportation for local travel between facilities
- Wound Care Certification (preferred, with CEUs provided if needed within 90 days of hire)
- Experience with wound documentation and treatment planning
- Competitive Pay: $160,000 annually with bonus opportunities
