Family Health Navigator – Children's Wisconsin
Greenfield, WI $50,000.00-$
Job SummaryImprove health and birth outcomes for pregnant or parenting women, men and their infants by linking identified needs with available community resources. Partner with local community organizations to facilitate integrated service delivery. Work with medical and behavioral health professionals to ensure follow up and coordination of recommended medical management.
Essential Functions- 60% of the role is spent outside of the office, conducting home visits, meeting with community partners, and disseminating community health information.
- Engages in community outreach activities. Connects with residents of identified neighborhoods, follows up with families and disseminates social and medical information and resources regularly.
- Collects information regarding services provided and the needs of community residents in a systematic, continuous, and confidential manner.
- Documents direct resource delivery provided to residents and maintains computer-based records.
- Facilitates continuity of health care for participants by planning and exchanging information with physicians, other school and community health and social service agencies.
- Provides group based parenting classes to Healthy Start program participants.
- Provides health education information to community residents to facilitate positive health behavior change among the population.
- Assists with data collection and data entry.
- Participates in advocacy activities to support the program.
- Provides feedback and evaluation opportunities for program participants and incorporates those results into program improvement.

- Fulfills requirements for continuing education.
- Maintains office communications and participates in agency sponsored meetings when required.
- Actively participates in reflective practice supervision.
- High School graduate or Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED) required.
- Bachelor's Degree preferred.
- 1+ years working in community programs with families of diverse ethnic, cultural and socioeconomic background required.
- In lieu of work experience, would consider lived or volunteer experience preferred.
- Demonstrated knowledge of psychosocial, economic and cultural determinants of healthy growth and development. Working knowledge of current trends and developments in the prevention and family support field. Knowledge of the ecological theories and family support principles.
- Must use personal vehicle to transport children.
- Must complete both in-house car seat education and transporter training within one month of hire. Must re-train and maintain the transporter training biannually.
- Demonstrates skills in creative problem-solving, community engagement, outreach and has in-depth knowledge of the assets and challenges of the community to be served.
- Has the ability to work with medical professionals to ensure follow up and coordination of recommended medical management.
- Committed to work toward improving one’s own cultural competence, i.e. valuing diversity, recognizing personal limitations in one’s own skills and expertise, and having the desire to learn. Accepting of individual differences.
- Ability to establish trusting relationships and work with families and children who are dealing with safety issues and other related problems.
- Excellent judgment skills. Learns quickly when facing new problems and makes good decisions based on a mixture of analysis. Ability to make sound judgments, to use supervision effectively and to take independent action if needed to ensure child safety. Is cool under pressure and can effectively cope with change.
- Ability to effectively communicate information and respond to questions from diverse individuals, community groups and the general public. Relates well to all kinds of people and is approachable. Commands attention and can manage group process during presentations and family meetings.
