Preceptor Home Health & Hospice South is growing! We are a home health, hospice, and palliative care agency providing services in patient homes in 15 counties throughout Wisconsin. Patient homes include private residences, assisted living facilities, memory care units, and resident care apartment complexes. Preceptor strives to provide high quality care through our multifaceted programs including home health, hospice, and palliative care.

We have branch offices located in Germantown, Fond du Lac, Waukesha, Sheboygan, Appleton, Racine (Mount Pleasant), and Kenosha.

Full-Time, 32-40 hours per week, M - F

• Carry a current licensure in field of practice in the State of Wisconsin.
• Excellent communication skills with patients, families, facility staff, doctors, and teammates.
• Ability to work independently.
• Good organization and effective time management skills.
• Ability to oversee OTA staff.
• Willingness to travel to meet patient needs.
• Compliance with documentation standards and regulations.
• Prior 1 year home health experience preferred, but does not affect eligibility to apply.
• Current CPR Certification.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle.

  Full-Time, 32-40 hours per week.
• Compensation: $80,000 - $95,000.00.
• Sign-On Bonus: $5,000.00.
