Summary:
The CDCA Case Manager provides comprehensive support and coordination of care for clients facing SUD challenges. This role involves assessing client needs, developing individualized service plans, and collaborating with multidisciplinary teams to ensure the delivery of effective and client-centered services. The Case Manager is dedicated to facilitating clients' recovery and promoting their overall well-being and independence.
