We are currently seeking a full time day I for our Children's program located in Delaware County, IN. This position will provide skills and development training to assist patients and/or families in developing skills directed towards achieving developmental milestones and reducing psychosocial barriers. A Skill Builder provides case management services, working in conjunction with the clinical team for coordination of care.
A Skill Builder works with the patient on a day-to-day basis using professional judgment and discretion to implement the team-determined treatment plan.
Key Responsibilities:
- Assists let Therapist in developing, implementing and revising individual treatment plans; assures that services provided are specified in the treatment plan and monitors progress toward treatment goals.
- For adults, provides individual and group skills training and development to assist the patient and/or family in developing skills (i.e. Self-care, daily life management, interpersonal communication, parenting, illness management, anger management, positive coping skills, and problem-solving skills) directed toward reducing psychosocial barriers.
- For children, provides individual and group skills training and development to assist the patient and/or family in developing skills (i.e. Coping with body image changes, coping with hormonal‑induced emotional mood swings, self-care, daily life management, interpersonal communication, illness management, anger management, positive coping skills, and problem-solving skills) directed toward achieving developmental milestones and reducing psychosocial barriers.
- Provides case management services as identifiedonomi in the patient’s treatment plan.
- Works collaboratively with the Therapist and medical staff to ensure coordination of integrated care for the patient, including identification of health issues, compliance with treatment, and,..effective patient-to-medical-provider communication.
- Completes CANS/ANSA re‑assessments. ће Provides consultation to staff on specific cases involving coordination of care.
- M..maintains positive working relationships with staff of other agencies and institutions, homes, and facilities, and acts as a liaison between patients and agencies and/or institutions.
- Prepares and maintains clinical records, reports and forms and other administrative tasks as required.
- Participates in continuing education activities, remaining knowledgeable 💡in areas of expertise.
- High school diploma or its equivalent with 2 years paid behavioral healthcare experience or equivalent college coursework toward a human services or health-related degree required at minimum
- Associate or Bachelor's degree preferred
- A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance
