Optum Home & Community Care is expanding its House Calls program in the United States. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct annual in-home health assessments for Medicare Advantage and other plan members.
This is a non-prescribing, field-based role focused on improving outcomes through education and collaboration with PCPs. We offer flexible scheduling (8am–8pm, 7 days a week), no on-call, no weekends, and no holidays.
