We are seeking a dedicated and experienced Registered Nurse (Home Health) to join our client’s growing team in Mundelein, Illinois. This position offers the opportunity to provide compassionate, high‑quality care to patients in the comfort of their own homes. The ideal candidate is a clinically skilled RN with excellent communication abilities and a strong commitment to patient‑centered care.

• Deliver skilled nursing care and conduct comprehensive patient assessments in home settings
• Develop, implement, and update individualized care plans based on patient needs and physician orders
• Educate patients and families about medications, treatments, and disease management
• Monitor patient progress, documenting observations and interventions accurately and promptly
• Collaborate effectively with physicians, therapists, and other healthcare professionals to ensure continuity of care
• Maintain compliance with all agency policies, regulations, and professional standards

• Monday through Friday, daytime hours
• On‑call rotation includes 1 weekend per month, 1–2 on‑call days per month, and 2 holidays per year
• This is a Pay Per Visit position

• Local travel within the Mundelein area (travel reimbursement provided)

• Associate degree or higher in Nursing
• Active Registered Nurse (RN) license in Illinois
• Minimum of 1 year of professional RN experience (home health experience preferred)
• Current BLS or ACLS certification through the American Heart Association
• Valid driver’s license, reliable transportation, and current auto insurance

• Strong clinical assessment and documentation skills
• Ability to work independently and manage time efficiently
