Nurse Practitioner, Healthcare Nursing
Job Description
Job Overview
A growing geriatric endocrinology practice is expanding into 12 new nursing home and skilled nursing buildings across the Cincinnati area. It’s hiring one to two full-time Nurse Practitioners to launch them and build specialty diabetes and thyroid care from the ground up. As the primary clinical decision maker for a portfolio of five to six long-term care facilities, you’ll start in brand-new buildings and build your own patient panel, with no one else’s panel to inherit.
Your visits focus on diabetes and thyroid care, not the five or six issues packed into a typical primary care visit. A collaborating physician is always one call, text, Face Time, or email away. The ramp is gradual. You start with one to two facilities and grow to five or six. In a new building you’ll see about 7 to 10 patients a day until every resident has been seen, then settle into about 30 patients a day across your facilities.
The schedule is Monday through Friday with a flexible start time, no weekends, and no after-hours coverage. Endocrinology experience is a plus, not a requirement. The practice layers its endocrine training on top of your baseline diabetes and insulin knowledge.
Key Responsibilities
- Provide on-site diabetes and thyroid care to residents in nursing homes, skilled nursing facilities, and long-term care centers, including new patient visits and follow-ups
- Manage insulin and other diabetes and thyroid medications, and adjust treatment plans based on each resident’s progress
- Act as the primary clinical decision maker for your assigned facilities, consulting the collaborating physician when you need support
- Educate patients on proper diabetes care, and their families when patients can’t make decisions on their own
- Build working relationships with facility leadership, directors of nursing, and nursing staff, adapting your communication style to each building
- Follow the practice’s standardized processes while adapting to how each facility operates
- Represent the practice as an ambassador at every facility you serve
- Maintain accurate, timely clinical documentation
Qualifications
- Active Ohio Nurse Practitioner license (AGACNP, AGNP, AGPCNP, ANP, or FNP preferred)
- At least 1 year of experience as an NP in long-term care, skilled nursing, or post-acute care (2 years ideal). RN experience in long-term care does not substitute.
- Baseline working knowledge of diabetes and insulin management, and of thyroid conditions
- Able to work on-site and travel between facilities in the Cincinnati area
- Strong interpersonal skills and the ability to communicate across different personality types with facility staff, leadership, patients, and families
- A self-starter who works well independently, with strong critical thinking and problem-solving skills
- Endocrinology experience preferred, not required
Schedule & Compensation
- $130,000 to $150,000/year, based on experience
- Productivity bonus of $2,500 per quarter (up to $10,000/year) for exceeding 1,800 billable encounters in the quarter
- Starts as a 1099 contractor for the first 90 days, then converts to W-2 at the same salary
- Full-time | Monday to Friday | Flexible start time (start early, finish early) | No weekends | No after-hours coverage
- Follow-up visits average about 15 minutes, with up to 30 minutes for complex patients
- Clustered facilities: no more than 30 to 40 minutes’ drive to a building, and a second building the same day is nearby or on your way home
- Healthcare stipend covering half of your monthly private health plan premium, starting after 60 days, with help enrolling
- IRA retirement plan with company match
- 15 days PTO plus paid holidays
- $1,500 CME stipend
- Mileage reimbursement at $0.52/mile after the first 20 miles driven each day
