Licensed Practical Nurse, Home Health

Center Well Home Health is seeking a Licensed Practical Nurse (LPN) for our home health team.

Base pay range: $44,600 – $61,400 per year. Pay per visit/unit: $28.00 – $40.00.

• Provide professional nursing services under the direction of an RN in compliance with the state’s Nurse Practice Act, licensure and regulations.
• Perform skilled nursing interventions, preventive care and therapeutic education in accordance with the plan of care.
• Apply clinical knowledge to facilitate problem resolution and achieve individualized patient goals.
• Consult with RN Supervisor regarding plan changes and verbal orders from a physician as permitted by law.
• Maintain a systematic, goal‑driven approach to the nursing plan.
• Coordinate with physicians, clinical managers and the healthcare team regarding patient status and discharge planning.
• Maintain confidentiality and professional conduct regarding patient information.
• Ensure supplies and equipment needed for patient care are available and safely used.
• Document and maintain records as required; submit shift notes when services are rendered.

• Graduate of an accredited Licensed Practical Nursing program.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• At least two years of LPN/LVN experience, preferably in home health or hospice.
