As an LPN with Spring Creek Rehab, your responsibilities will include:

• Providing direct patient care to residents, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs)
• Collaborating with our interdisciplinary team to develop and implement individualized care plans, ensuring that each resident receives the highest level of care
• Maintaining accurate and detailed records of patient care, including documentation of treatment plans, progress notes, and medication administration
• Participating in quality improvement initiatives to continuously improve patient outcomes and resident satisfaction

• Develop and maintain a positive, supportive, and professional attitude, reflecting positively on the organization
• Participate in mandatory in-service education programs and other training initiatives to maintain current licensure and stay up-to-date with industry best practices
• Demonstrate a commitment to excellence in patient care, customer service, and teamwork

• Current LPN licensure in the state of Kentucky or Multsitate Compact Licensure
• We welcome new grads
