Starting Salary Range: $24 per hour

What’s the role?

Community Health Workers (CHWs) at Novum Health are trusted members of the community who help individuals navigate challenges with mental health, substance use, and social barriers. CHWs meet people where they are, whether at home, in the community, or in recovery settings. Building trust, providing support, and connecting members with care and resources.

Responsibilities
  • Assess the healthcare needs of individuals within the community and identify gaps in care.
  • Collaborate with healthcare providers, social service agencies, and community organizations to develop comprehensive care plans.
  • Assist patients in navigating the healthcare system and accessing necessary services, such as medical appointments, specialist referrals, diagnostic tests, and community resources.
  • Coordinate and schedule appointments for patients with multiple providers to ensure continuity of care.
  • Facilitate communication between patients, providers, and community partners.
  • Monitor patients’ progress and adherence to care plans, providing support and education to promote self-management and prevention.
  • Conduct home visits or outreach activities to assess patients' living conditions, social support systems, and adherence to treatment plans.
  • Advocate for patients’ needs and rights within the healthcare system and community.
  • Document patient interactions, care plans, and progress notes in electronic health record and care management systems.
  • Participate in interdisciplinary team meetings and case conferences to ensure effective coordination and collaboration.
  • Provide transportation with personal vehicle for members to appointments when needed (mileage reimbursed).
  • Perform additional duties as assigned.
  • Build strong relationships with individuals and families to promote trust and reduce stigma.
Qualifications
  • Comprehensive knowledge of healthcare systems, community resources, and social services.
  • Knowledge of behavioral health, substance use treatment, and community resources.
  • Strong interpersonal and communication skills to establish rapport.
  • Excellent organizational and time management skills.
  • Ability to assess patient needs, develop individualized care plans, and evaluate outcomes.
  • Proficiency in using electronic health records and care management software.
  • Understanding of privacy regulations and compliance requirements.
  • Compassionate and patient-centered approach.
  • Problem-solving and critical thinking skills.
  • Cultural competence and sensitivity to diverse populations.
  • Team collaboration and positive relationships with healthcare professionals and partners.
What you’ll need to start
  • At least 1 year of experience in community health, behavioral health, peer support, or related roles.
  • Bachelor’s degree in healthcare, social services, or related field preferred.
  • An active and valid Community Health Worker Certification.
  • CPR/First Aid (or willingness to obtain).
  • Pass drug screening and background check.
  • Independent transportation vehicle to assist with member transport (mileage reimbursed).
  • Willingness to receive vaccinations per hospital and state requirements.
