** Join Mission Australia as a Case Manager and make a real impact by supporting clients with complex needs to achieve positive, sustainable  these roles, you will deliver person-centred case management using evidence-based approaches, create holistic plans tailored to individual strengths, and guide clients through transitions toward independence. Working under general supervision, you will apply specialist knowledge and sound judgment to provide high-quality support while building strong relationships with internal teams and external partners.

Your key responsibilities will be to:
* Deliver evidence‑based therapeutic interventions, manage a complex caseload, monitor progress and adjust plans as needed.
* Support client transitions to independence or other services, building capacity for self‑management; provide outreach where required.
* Prepare accurate case notes, maintain client files and enter data promptly in relevant client management systems.
* Compile internal and external reports (e.g., case statistics, feedback summaries) and contribute to program delivery.
* Develop, coordinate and facilitate group programs or training activities in line with program guidelines.
* Actively contribute to continuous quality improvement and the development, implementation and review of service practices.
* Build sustainable relationships with clients, colleagues, service providers and government agencies; participate in network meetings and share insights.
* Provide guidance, mentoring and coaching to employees at lower levels; share specialist advice and best practice.
** Requirements for success
*** Relevant tertiary qualification in social work, community services, psychology or similar;
** or
* * an associate diploma with relevant experience;
** or
* * equivalent industry experience.
* Demonstrated experience delivering case management to clients with complex needs, including evidence‑based interventions and managing a complex caseload.
* Demonstrated ability to partner with clients to develop individualized, strengths-based plans, identify meaningful goals, and support progress through coordinated referrals and regular review.
* Experience conducting intake, registrations and formal assessments; maintaining accurate case notes and timely data entry in client management systems.
* Strong engagement skills and the ability to build and maintain effective relationships with clients, colleagues, service providers and government agencies.
* Well‑developed written and verbal communication skills, plus high‑level administrative and reporting capability.
* Ability to develop, coordinate and facilitate group programs or training in line with program guidelines.
* Current valid WWCC, First Aid & CPR and a driver’s licence are essential to the role Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
