• Assess patients’ functional status and PT needs in their homes.
• Develop and implement individualized care plans.
• Provide direct PT services in accordance with the physician’s plan of care.
• Educate patients, families, and caregivers on exercises and therapy techniques.

• Current licensure as a PT in Michigan.
• Graduate of an accredited PT program.
• At least one year clinical experience (home care preferred).
• Valid driver’s license and reliable transportation.
• Ability to work independently and manage time effectively.
