* Responsibilities:
* * Assess patients' functional status and physical therapy needs in their homes.

* Develop and implement individualized care plans.

* Provide direct physical therapy services in accordance with the physician's plan of care.

* Educate patients, families, and caregivers on exercises and therapy techniques.

* Qualifications:

* * Current licensure as a Physical Therapist in the State of Michigan

* Graduate of an accredited physical therapy program.

* At least one year of clinical experience (home care experience preferred).

* Valid driver's license and reliable transportation.

* Ability to work independently and manage time effectively.
