Under the direction of the RN Supervisor, the Registered Nurse plays a vital role in partnering with patients, families, facilities, and care teams to support the management of mild to chronic conditions and complex illnesses. This role focuses on providing coordinated, patient-centered care across the continuum and between care settings.
The RN engages patients in the assessment, planning, monitoring, and evaluation of their ongoing care needs. This includes developing individualized care plans, supporting self-management, delivering patient education, and ensuring appropriate and efficient use of resources. The RN is accountable for continuously monitoring patient progress and outcomes.
Additional responsibilities include population health management, care coordination, patient education, and oversight of patient registries. The RN works proactively to help patients optimize their health, reduce preventable hospitalizations, and avoid unnecessary emergency room visits.

- Engage patients in assessment, planning, monitoring, and evaluation of chronic and complex condition management
- Review clinical documents and develop comprehensive plans of care, including patient education and self-management support
- Coordinate resources to ensure effective, efficient, and appropriate patient care
- Monitor and evaluate patient outcomes and document progress
- Perform population health functions, including registry oversight, education, and care coordination
- Support patients in improving health status and preventing avoidable hospital or ER visits
- Graduate of an accredited nursing program (ASN required; BSN preferred)
- Current, active Registered Nurse (RN) license in good standing with the State of Michigan
- Three or more years of clinical experience preferred; strong organizational, interpersonal, and analytical skills required
- Current BLS certification
- Knowledge of chronic conditions, evidence-based guidelines, prevention, wellness, health assessments, and patient education
