• Assist the Charge Nurse/RN by mentoring and directing CNA staff to ensure the highest standard of daily resident care.
• Perform and document patient evaluations, recognizing subtle changes and reporting immediate interventions to the Charge Nurse/RN.
• Contribute to development, implementation, and direction of personalized care goals focused on maximizing resident independence and recovery.
• Collaborate with RNs, physicians, therapists, and social workers to coordinate holistic, multidisciplinary care strategies.
• Safely and accurately administer medications, treatments, and procedures as prescribed.
• Champion patient safety by adhering strictly to all safety, infection control, and environmental standards.

• Current and unrestricted State LPN license in Michigan.
• Commitment to representing Optalis with integrity and upholding our core values.
• Strong communication and teamwork skills to support CENA/CNA staff.
• Proficiency with or ability to quickly master EHR and clinical documentation systems.

• Prior experience (1+ year) in LTC or SNF setting.
• Knowledge and adherence to current Infection Control protocols and best practices.
