What You’ll Do in This Role
  • The Life Circles clinical team delivers primary care, palliative care, and end-of-life care for patients aged 55 and older. This position is perfect for the healthcare provider who wants to be patient-centered, creative, and collaborative in their approach to care.
  • The successful candidate will join an experienced Interdisciplinary Team and work closely with other Advanced Practice Providers and healthcare professionals in a well-supported, team-based environment.
Primary Responsibilities
  • Participating in daily interdisciplinary team meetings
  • Completing periodic health assessments
  • Assessing and treating acute needs and illness
  • Advanced Care Planning
  • Patient and Caregiver Education
What You’ll Need
  • Current license and certification to practice as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Michigan
  • Master’s degree in nursing or satisfactory completion of a physician assistant program
  • Active Registered Nurse (RN) license in the state of Michigan.
  • CPR certification or willingness to obtain
  • Degree in Nursing;
    bachelor’s degree in nursing (BSN) preferred
  • Minimum of 1 year of nursing experience.
  • Previous experience in home health, case management, or a related field preferred.
  • Current DEA registration
  • State of Michigan Controlled Substance License
  • Minimum of 1-year experience with the frail and elderly or long-term care population preferred
