. We’re looking for a dedicated RN with a weekend schedule who wants to make a meaningful impact in home health.

As a Home Health Registered Nurse, you’ll provide in‑home skilled nursing care to patients by developing personalized care plans, administering medications, and supporting recovery and independence. Whether you’re experienced in home health or coming from an acute care setting, this role offers an opportunity to apply your clinical expertise in a patient‑focused environment.

Full time – Shift: Saturday to Monday.

• Provide skilled nursing care, assessments, and interventions in the home
• Serve as case manager and patient advocate
• Collaborate with interdisciplinary care teams
• Educate patients and families on care plans and self‑management

• Active RN license in Michigan
• 2+ years of clinical nursing experience (Home Health preferred)
• Epic or Home Care EMR experience
• Strong communication, assessment, and organizational skills
• Compassionate, dependable, and mission‑minded
• Valid driver’s license and reliable transportation required
