Position Overview

The Nurse Practitioner (NP) provides high‑quality, patient‑centered primary care to individuals under the supervision of a licensed physician. Practicing within a Federally Qualified Health Center (FQHC), the NP supports improved health outcomes through clinical excellence, patient education, and primary care, with a commitment to serving diverse populations in West Michigan. This role requires adherence to professional standards, licensing regulations, and internal policies, including ethical conduct and confidentiality.

Duties And Responsibilities
  • Conduct comprehensive health assessments and physical exams, including obtaining patient histories.
  • Diagnose and manage acute and chronic illnesses using evidence‑based practices.
  • Order, interpret, and follow up on diagnostic tests including laboratory studies, imaging, and EKGs.
  • Perform and oversee therapeutic procedures such as immunizations, wound care, and minor in‑office procedures within scope of practice.
  • Develop and manage patient‑centered treatment plans, prescribe medications within the NP’s scope and Michigan state regulations.
  • Educate patients and families on disease prevention, treatment compliance, and health maintenance.
  • Accurately document all clinical encounters in the electronic medical record (EMR).
  • Collaborate with interdisciplinary team members, including referrals to specialists as needed.
  • Participate in clinical meetings, peer reviews, and quality assurance initiatives.
  • Support FQHC initiatives including compliance with HIPAA, OSHA, HRSA, and UDS standards.
  • Demonstrate professionalism and empathy in all patient and staff interactions.
Education and Licensure
  • Graduate of an accredited Master’s or Doctoral Nurse Practitioner program.
  • Current and unrestricted licensure as a Registered Nurse (RN) and Nurse Practitioner (NP) in the State of Michigan.
  • National certification as a Family Nurse Practitioner (FNP), Adult‑Gerontology NP, or other applicable certification through AANP or ANCC.
  • Current DEA registration (or eligible to obtain).
  • Basic Life Support (BLS) certification; ACLS preferred.
  • Compliance with state and federal continuing education requirements for licensure and credentialing.
Required Knowledge, Skills, And Abilities
  • In‑depth knowledge of primary care and chronic disease management.
  • Familiarity with diagnostic and therapeutic clinical procedures.
  • Skilled in maintaining EMR documentation, medical terminology, and reporting.
  • Ability to provide culturally competent, patient‑focused care.
  • Ability to lead health promotion and disease prevention initiatives.
  • Strong interpersonal skills and ability to work in a collaborative care team.
  • Knowledge of patient rights, ethics, and confidentiality practices.
