Sage Group Health Care  Staffing is seeking Hospice Social Worker for a 13-week travel /contract position in Muskegon, OK. Accepting both local and traveler candidates.

Pay Rate:

Range $1900 to $2000 Weekly Gross pay (Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.50/mile for distance covered between patient home appointment

Unit: Home Health Hospice

Shift: 8 Hours;
Days, 8a – 5p

• Social Work experience in ahealthcare setting: 1 year - Required
• Hospice Social Work experience - Highly preferred
• Willing and comfortable to visit patient homes - Required
• Must have valid Driver’s License and Personal Auto Insurance
• Must be Covid Vaccinated orprovide medical or religious exemption

• Bachelor’s degree in Social Workor equivalent – Required
• Master’s Degree in Social Work or equivalent – Highly preferred
• Social Work License (LBSW or LMSWor LCSW or LICSW) or equivalent – Required
• BLS Certification from AHA or American Red Cross (Required)

• Explains hospice services and Medicare benefits to patients and families; obtains Informed Consent and Election of Benefits documents as requested.
• Submits appropriate documentation and paperwork to facilities at the completion ofpatient visits; documentation is completed and submitted timely according tocompany policy.
• Participates as amember of the Interdisciplinary team, including development and implementation of the plan of care.
• Provides psycho-social support and counseling services to the patient and family;supports the involvement of external counseling resources commensurate with patient/family needs and scope of practice.
• Completes initial and ongoing psycho-social assessments for patients; identifies and addresses concerns/ issues related to financial means, insurance, living arrangements/placement for long-term care, caregiver stressors, pre-bereavement and anticipatory grief, and community resource needs.
• Supports and facilitates advance planning including living will/ POA documents, funeral arrangements, memorial services, and body/ organ donation, etc.,
• Performs other duties as assigned
