• 30 days PDO (up to 6 weeks, including company holidays)

• Perform skilled nursing visits and coordinate client care
• Act as point of contact for all disciplines involved in patient care
• Oversee frequency of visits for the episode
• Consult with the physician and office on patient care details
• Accurately collect OASIS data, ensuring medication profiles remain current
• Ensure lab values are reported to the physician in a timely manner
• Attend weekly case conferences and monthly case manager meetings

• Registered Nurse licensed in the state of employment
• Minimum of one year of clinical experience preferred
• Demonstrated knowledge and skill in current nursing practice
• Valid state driver’s license and automobile liability insurance
