• Provide outpatient rehabilitation services in in‑home settings for older adults, evaluating and delivering individualized PT programs.
• Assess patients, develop treatment plans, and regularly re‑examine to adjust care.
• Instruct patients and caregivers on exercises, functional training, manual therapy, airway clearance, and health education.
• Collaborate with interdisciplinary teams, make referrals, and coordinate comprehensive care.
• Maintain safe environment, order and maintain supplies, and document clinical and billing information.
• Supervise PT assistants, students, and support personnel as required.

• Valid Physical Therapy license in the state(s) of practice or proof of license eligibility.
• Master’s degree in Physical Therapy or a related field, or Bachelor’s degree with 5 years progressive PT experience.
• U.S. equivalent evaluation for non‑U.S. degrees.
• Valid driver’s license or reliable transportation to patient residences.
