Een maatschappelijke organisatie in Naaldwijk zoekt een Wmo-consulent die het verschil wil maken in het leven van mensen. In deze rol ben je het aanspreekpunt voor inwoners met een hulpvraag en help je hen bij het zelfstandig wonen en meedoen in de maatschappij. Je voert gesprekken, beoordeelt hulpvragen en stelt rapportages op. Ervaring met het ondersteunen van mensen met diverse hulpvragen is gewenst.
