Licensed Social Worker, Full Time

Avir at Nacogdoches is seeking a compassionate, organized, and resident-focused Licensed Social Worker to join our team full time. This position plays an essential role in supporting the psychosocial well-being, rights, needs, and quality of life of our residents while working closely with residents, families, interdisciplinary team members, and community resources.

What You'll Do:
  • Assess residents' psychosocial needs and participate in the development and implementation of individualized care plans
  • Provide support, advocacy, counseling, and resource coordination for residents and their families
  • Assist residents and families with adjustment to placement, changes in condition, care planning, and other concerns related to long-term care or rehabilitation
  • Participate in interdisciplinary care plan meetings and communicate resident needs and concerns to the appropriate team members
  • Coordinate discharge planning and assist with referrals for community services, equipment, transportation, home health, hospice, and other resources as appropriate
  • Assist residents with advance directives, resident rights, grievances, and other psychosocial or social service needs
  • Identify and address behavioral, emotional, social, and environmental factors that may affect a resident's overall well-being
  • Maintain timely, accurate, and confidential documentation in accordance with facility policies and applicable state and federal requirements
  • Assist with resident and family concerns and help facilitate effective communication between residents, families, staff, and facility leadership
  • Participate in resident assessments, including applicable sections of the Minimum Data Set (MDS), care planning, and required documentation within the scope of the position
  • Support resident rights, dignity, self-determination, and freedom from abuse, neglect, exploitation, and discrimination
  • Make appropriate referrals and coordinate services with outside agencies and healthcare providers as needed
  • Participate in facility meetings, quality initiatives, surveys, and other interdisciplinary activities as assigned
About Our Ideal Candidate:
  • Current Texas social work license in good standing (LBSW, LMSW, or LCSW, as applicable)
  • Degree in Social Work from an accredited college or university
  • Previous experience in a skilled nursing facility, long-term care, rehabilitation, healthcare, or geriatric setting preferred
  • Knowledge of resident rights, psychosocial care, discharge planning, and community resources
  • Familiarity with Medicare, Medicaid, MDS, and long-term care regulations is preferred
  • Strong communication, documentation, organization, and problem-solving skills
  • Ability to work collaboratively with residents, families, nursing staff, physicians, therapy, administration, and other members of the interdisciplinary team
  • Compassionate and professional approach when working with elderly, disabled, medically complex, or emotionally distressed residents and families
  • Ability to maintain confidentiality and handle sensitive situations with professionalism and discretion
